Any residual balance or non-covered items will be billed directly to you. Please notify the administrative staff as soon as possible if there is any change to your personal or insurance information as this will help us to process your claim quickly and avoid unnecessary bills.

Medical Records

Medical records are available upon request. Any records requested by a physician will be sent the same day. Requests by non-physicians (including patients) will be processed and sent via mail on Fridays.

All legal and personal requests for records have a processing fee depending on the size of the chart and number of copies necessary. The fee schedule is dictated by state law and is available on request.

Our highly-qualified nursing staff is here to assist you through every step of your office visit, from taking your patient history and recording your vital signs, to performing special tests and assisting physicians with preparations and procedures, to taking care of your prescription needs.

Nurses are available to answer phone messages and e-mails regarding medication requests and medical questions and concerns. Some inquiries may require a conference with your physician and in that case we will accommodate you as best as possible. Several nurses are also certified in intense pulsed light (IPL) therapy for skin and hair treatment.

You can ask to see them separately, or at any time during your visit. The fee schedule is based on areas of treatment and is available on request.
